KV Pharmaceutical Announces Recall of Hydromorphone

morphine isosorbide propafenone December 23, 2008—KV Pharmaceutical announced it would recall Hydromorphone, after finding the drug may contain double the dose. This recall is the third in a string of acknowledgments that manufacturing defects have resulted in production of drugs with double strength.

ETHEX Corporation Expands Recall

 

November 10, 2008—ETHEX Corporation expanded its recall to include Morphine Sulfate, Isosorbide Mononitrate and Propafenone HCl tablets. The recall is the result of manufacturing defects that resulted in production of tablets with double strength.

The expanded recall now includes the following tablets within specific lots:

  • 5- and 10-mg dextroamphetamine sulfate tablets
  • 150-, 225- and 300-mg propafenone HCl
  • 30- and 60-mg isosorbide mononitrate extended-release tablets
  • 15-mg morphine sulfate extended-release tablets
  • 15- and 30-mg morphine sulfate immediate-release tablets

Manufacturing Defect Leads to Recall of Dextroamphetamine by ETHEX Corporation

 

 

October 15, 2008—ETHEX Corporation announced a recall of Dextroamphetamine after determining that specific lots of the tablets may contain twice the approved level of the active ingredient.

The recalled lots are 77946, 81141 and 81142 and were distributed between January 2007 and May 2008. The tablets are orange and identified by the markings "ETHEX" and "311."

Side Effects of Dextroamphetamine

Ingesting pills produced with double strength may increase side effects associated with Dextroamphetamine. Those side effects and symptoms include:

  • Tachycardia (rapid heart beat)
  • Hypertension
  • Tremors
  • Decreased appetite
  • Headache
  • Insomnia
  • Dizziness
  • Blurred vision
  • Upset stomach
  • Dry mouth
